The short version: every U.S. state and DC requires state-regulated health plans to cover ABA/autism services — Indiana was first (2001) and the final holdouts were resolved by 2019 — and in every state, medically necessary ABA is covered for children under Medicaid's EPSDT benefit. What differs by state is the program names, the plan types the mandate reaches, and the early-intervention system for children under 3.
